Navigation is missing within Management Reporter

After a recent deployment of Management Reporter 2012, users reported having no navigation available within Report Designer.  The toolbar and menus were accessible.  However, the navigation pane known as the Go Menu in Report Designer only displayed Security.

Attention! SQL Backup Failed

When attempting to take a backup of a GP database in SQL Server Management Studio, you receive the error: 

Backup failed for Server ‘SERVERNAME’

Additional Information: 
An exception occurred while executing a Transact-SQL statement or batch.  (Microsoft .SqlServer.ConnectionInfo)

Failed to flush the commit table to disk in dbid # due to error 2601.  Check the errorlog for more information.  (Microsoft SQL Server, Error:  3999)

Browser message generating Management Reporter reports

This report can't be displayed

Make sure that you are set up in Management Reporter as a user.

With Cumulative Update 6 (CU6) for Management Reporter 2012 released, now is a great time to check if your Management Reporter installation is current?  Is your business taking advantage of the enhancements Microsoft has released within the updates?

One might first ask “How can I check the version?” To determine the version of Microsoft Management Reporter, start Microsoft Management Reporter. On the Help menu, click About Management Reporter.

Error – The Operation could not be completed due to a failure on the server.

Scenario –
Upon completing a Management Reporter 2012 Rollup 4 deployment with Hotfix, the installation was validated as functional and ready for the migration of FRx reports. Migrating the reports without any concerns, it was time to start validating the definitions within Management Reporter. Launching Management Reporter, the following error displayed –

‘The operation could not be completed due to a failure on the server.’

Checking the Configuration Console for Management Reporter, the Service logs captured the following –
